Primos Butchershop Pro Wrestling Gym provides professional wrestling training designed to teach discipline, body control, physical conditioning, wrestling fundamentals, safety, confidence, teamwork, and performance skills. Because professional wrestling training involves physical activity and contact, there are inherent risks of injury. This form must be completed and signed by a parent or legal guardian before a minor may participate in training. 1. ACKNOWLEDGMENT OF TRAINING RISKS I, the undersigned parent or legal guardian, understand that professional wrestling training is a physical activity that may involve strenuous exercise, physical contact, falls, rolls, lifting, grappling, running, jumping, tumbling, conditioning drills, use of wrestling mats and other gym equipment, and supervised wrestling exercises. I understand that, even when proper instruction, supervision, safety procedures, and equipment are used, participation may involve risks including, but not limited to: ⦁Sprains, strains, bruises, cuts, scrapes, and muscle soreness; ⦁Falls and accidental collisions; ⦁Injuries to the head, neck, back, shoulders, arms, hands, hips, knees, ankles, and other parts of the body; ⦁Concussions or other head injuries; ⦁Broken or fractured bones; ⦁Joint, ligament, tendon, or muscle injuries; ⦁Serious bodily injury; ⦁Medical emergencies; and ⦁In rare circumstances, permanent disability or death. I understand that professional wrestling is a physically demanding activity and that no training program can completely eliminate the possibility of injury. 3. VOLUNTARY PARTICIPATION I voluntarily give permission for my child to participate in training activities at Primos Butchershop Pro Wrestling Gym. I understand that my child will be expected to follow the instructions of the coaches and trainers, follow gym rules, use equipment appropriately, and behave respectfully toward coaches and other participants. I understand that coaches may modify, restrict, or stop an activity when they believe doing so is necessary for safety. I agree that my child will immediately inform a coach or trainer if they experience pain, dizziness, illness, injury, or any other physical concern during training. 4. MEDICAL CONDITION AND FITNESS TO PARTICIPATE I represent that, to the best of my knowledge, my child is physically able to participate in the activities for which I am giving permission. I agree to inform Primos Butchershop Pro Wrestling Gym of any medical condition, injury, allergy, physical limitation, medication, or other circumstance that could reasonably affect my child’s ability to safely participate. Medical conditions or restrictions the gym should be aware of: Allergies: Medications or other important medical information: I understand that the gym and its coaches are not responsible for determining whether my child is medically fit to participate. I may consult my child’s physician before participation if I have any concerns. 5. EMERGENCY MEDICAL AUTHORIZATION In the event of an emergency involving my child, I authorize Primos Butchershop Pro Wrestling Gym, its coaches, trainers, staff, and designated representatives to seek reasonable emergency medical assistance for my child when I cannot be reached promptly. I understand that the gym will make reasonable efforts to contact me or another emergency contact as soon as reasonably possible. RELEASE AND WAIVER To the fullest extent permitted by applicable law, I, on behalf of myself and my child, release and hold harmless Primos Butchershop Pro Wrestling Gym, Primos Pro Wrestling LLC, and their owners, operators, coaches, trainers, employees, volunteers, contractors, agents, representatives, and affiliated personnel from claims arising from my child’s voluntary participation in gym activities, including claims for personal injury or property damage resulting from the inherent risks of the activity. This release does not apply to conduct that cannot legally be released or waived under applicable law, including liability that may not lawfully be excluded or limited. I understand that this section is intended to be interpreted only to the extent permitted by Colorado law. 7. INDEMNIFICATION To the fullest extent permitted by applicable law, I agree to indemnify and hold harmless the parties identified above from claims, damages, or expenses arising from my child’s participation in gym activities to the extent such indemnification is legally enforceable. I understand that this provision does not require me to indemnify any person for conduct that cannot legally be indemnified or released. 8. RULES AND CONDUCT I understand that my child must follow all reasonable gym rules and instructions. I acknowledge that Primos Butchershop Pro Wrestling Gym may remove a participant from training if the participant: ⦁Repeatedly disregards safety instructions; ⦁Intentionally engages in unsafe conduct; ⦁Harasses, threatens, or bullies another participant; ⦁Uses equipment improperly; ⦁Engages in behavior that creates an unreasonable safety risk; or ⦁Otherwise violates reasonable gym policies. I understand that wrestling training is a team environment and that respect for coaches, other students, parents, and gym personnel is expected. 9. PARENT/GUARDIAN RESPONSIBILITY I understand that I am responsible for providing accurate and current information regarding my child. I agree to notify the gym promptly if my contact information, emergency contact information, or my child’s medical information changes. I understand that parents/guardians should communicate with the coaches regarding injuries, medical restrictions, or other circumstances that may affect their child’s participation. 10. PERSONAL PROPERTY I understand that my child is responsible for personal belongings brought to the gym. To the fullest extent permitted by law, the gym is not responsible for lost, stolen, or damaged personal property. 11. ACKNOWLEDGMENT AND AGREEMENT By signing below, I confirm that: ⦁I am the parent or legal guardian of the minor identified in this form. ⦁I have read and understand this entire agreement. ⦁I have had an opportunity to ask questions regarding the training program and the risks involved. ⦁I understand that professional wrestling training involves inherent physical risks. ⦁I voluntarily give my child permission to participate in training at Primos Butchershop Pro Wrestling Gym. ⦁I understand that my child is expected to follow the instructions of the coaches and trainers. ⦁I understand that this agreement is intended to be legally binding to the fullest extent permitted by applicable law. ⦁I have provided truthful and accurate information to the gym. Primos Butchershop Pro Wrestling Gym should have a Colorado attorney review this form before using it with minors, particularly the release, indemnification, emergency medical authorization, and any provisions concerning parental rights or minors. I’d also recommend having a separate Youth Registration & Information Form that collects the child’s birthday, parent information, emergency contacts, medical information, allergies, authorized pickup people, and training rules. That keeps the liability waiver itself cleaner and makes the packet easier for parents to complete.
